If you’re living with Hashimoto’s or Hypothyroidis If you’re living with Hashimoto’s or Hypothyroidism, hunger, cravings and energy can feel unpredictable. One moment you’re fine… the next you need food (or coffee, or chocolate!) right now.

But not all “hunger” signals actually mean you need food.

Some come from hormonal imbalances and others are simply your body asking for water.

- Ghrelin rises when your stomach is empty, and thyroid dysfunction can make these spikes more sudden.
- Leptin, your satiety hormone, can be disrupted by inflammation, leading to eating enough but still feeling hungry.
- Cortisol spikes (common with blood sugar swings and stress) can trigger cravings for carbs, sugar and caffeine.

And then there’s the most overlooked factor: dehydration.

Your hypothalamus controls both hunger and thirst, so mild dehydration can feel like hunger or cravings, especially for salty or sweet foods. 

Many thyroid patients are more prone to dehydration due to slower metabolism, constipation, fatigue, increased medication needs and caffeine use.

Before giving in to a craving:

- Drink a glass of water and wait 10–15 minutes.
- If the “hunger” fades, it was dehydration.
- If it stays, your body genuinely needs nourishment.

This simple habit can prevent overeating, stabilise blood sugar and reduce cravings, especially evening snacking, which often stems from under-hydrating all day.

Some supportive habits you can include in your daily routine:

- Water before coffee every morning
- Protein with every meal to balance blood sugar & grehlin, reducing cravings
- Anti-inflammatory foods too support leptin sensitivity
- Steady hydration (6–8 cups/day)
- A pinch of sea salt & a slice of lemon in water each morning for electrolytes
- Notice if hunger is tied to stress or fatigue

Your hunger hormones aren’t broken, they’re communicating.

With awareness and proper hydration, you can reduce cravings and support more stable energy.

Your body needs support, not shame.

Perimenopause + Hashimoto’s = chaos If you’ve not Perimenopause + Hashimoto’s = chaos

If you’ve noticed your once-stable thyroid levels or antibodies suddenly going wild… you’re not imagining it. Perimenopause can send everything off balance — thyroid hormones, autoimmunity, mood, sleep — the lot.

💡 Hint: It’s usually a mix of all three: thyroid, autoimmunity and perimenopause.

And the drop in oestrogen + progesterone is often the driving force.

But here’s the good news…

The FDA has removed the black box warning from many HRT products — a label that scared women off HRT for 20+ years. This is HUGE.

It doesn’t mean HRT is right for everyone, but for most women (especially under 60 or within 10 years of menopause), the benefits often outweigh the risks:

✨ Better sleep
✨ Mood support
✨ Fewer hot flashes
✨ Stronger bones
✨ Heart & brain benefits

And YES — this can apply if you have Hashimoto’s or Hypothyroidism.

HRT isn’t contraindicated, and many women actually feel more stable once their oestrogen is supported. It helped stabilise my own thyroid levels and massively improved symptoms like migraines, joint pain, low mood, brain fog and fatigue.

A few quick thyroid/HRT tips:

• Oral oestrogen can increase your thyroid medication needs.
• Transdermal oestrogen (patch/gel/spray) is usually better for thyroid patients.
• Symptoms overlap — testing is key.
• Better sleep + less stress = fewer autoimmune flares.

Bottom line:

With the black box warning gone, it may be worth revisiting if you’ve been told “it’s not safe for you” because of your thyroid.

And remember: HRT isn’t a magic fix. If stress, inflammation, diet or habits are out of balance, those still need love too. But with the right support, it can make midlife so much more manageable.
